Dementia and Caregiver Support Services

The agency aims to support people with dementia and their caregivers to ensure the highest quality of life possible while living in their homes. This program addresses the financial hardships, emotional stressors, employment issues, and other conflicts experienced by caregivers who provide around-the-clock care at home. The scope of work covers support and services for family members, friends, and others who provide in-home care for older adults, older relative caregivers of children, and caregivers of adults with severe disabilities. The selected provider must deliver these services offsite in alignment with the agency priorities and program frameworks.
