RFI for Case Management and Housing Placement Services

The agency is seeking information to identify vendors capable of providing comprehensive case management and housing placement services to veterans. The primary objective is to meet the clinical and housing needs of the served population through a multidisciplinary team approach. The agency recommends utilizing a team that includes mental health and addiction treatment professionals, social workers, nurses, psychologists, housing specialists, and peer specialists. The scope of work requires delivering most services directly in the community or within a veteran's home. Additionally, the selected team must visit veterans during periods of hospitalization or incarceration. Staff will also accompany veterans to clinics, emergency visits, and other appointments as necessary to ensure continuity of care. The agency limits eligibility to onshore organizations based in the United States. All performance of the work will occur offsite rather than at agency facilities.
