Community Health Worker Program Evaluation Consultant Services

The Riverside County Department of Public Health in California is seeking community health worker program evaluation consultant services. The agency relies on community health workers to advance healthcare access, provide health education, support care coordination, and connect residents to public health and social service resources. To optimize these efforts, the agency needs to evaluate and improve its program infrastructure, enhance service delivery, and ensure long-term program sustainability. The scope of work covers evaluating community health worker activities across multiple clinic-based and community-based programs. These activities include outreach, chronic disease prevention, injury prevention education, health navigation, and services addressing social determinants of health. Bidders must submit a sample evaluation plan, framework, or tools demonstrating familiarity with mixed methods evaluation, data management, and program indicator analysis. The evaluation must assess client satisfaction, assessments, community health worker reports, program reach, training competencies, client-level outcomes, and program sustainability. Eligible bidders must be onshore organizations based in the United States. The agency allows the work to be performed either onsite in Riverside, California, or offsite. The agency is currently looking for proposals to determine the budget for this project. A non-mandatory pre-proposal conference will take place on June 16, 2026.
