Reach Crisis Care Coordination Services

The government authority located in Wisconsin is seeking a vendor to provide Reach Crisis Care Coordination services. The program, known as REACH (Reaching, Engaging, & Assisting Children/Families), assists with conflict management to prevent out-of-home placement, improves youth coping skills to reduce risk of harm to self or others, and assists in obtaining supportive community services. The scope of work requires a Care Coordinator to assist youth with developing a crisis plan and connecting to support and services within the community or the youth’s insurance. Program enrollment may also include crisis stabilization and some time-limited specialized services. The performance of this work will occur offsite. Eligible youth for these services must be between the ages of 5 and 23, must be Medicaid-eligible, and must have received a mental health diagnosis within the past twelve months. Because REACH is a voluntary program, eligible youth must also desire to be enrolled in the services. Bidders must be onshore USA organizations to be eligible for this opportunity. The agency is looking for budget proposals to determine the cost of the project. All questions regarding this opportunity must be submitted no later than July 2, 2026.
