RFI for Outpatient Remote Monitoring (RPM) Services

The West Virginia government authority is seeking information to identify vendors capable of providing outpatient remote monitoring services. The agency aims to proactively monitor and manage patients in home and community settings to reduce avoidable emergency department visits and hospitalizations. Additionally, the agency wants to improve health outcomes for rural residents with chronic conditions and bridge care gaps by deploying scalable, cost-effective, and user-friendly solutions. The project scope requires proposing models that can remain sustainable beyond the initial grant period. The agency is looking for integration with value-based care arrangements and the State's evolving data infrastructure, specifically the Connected Care Grid Data Spine and Health Information Network. Performance of the requested work will take place both onsite and offsite. Eligible respondents must be onshore organizations based solely in the United States. The agency is currently looking for proposals to understand budget requirements and has not disclosed a set funding limit.
