Community Breast, Cervical and Colon Prevention Program Services

The Chicago-based government authority is seeking a vendor to deliver community breast, cervical, and colon prevention program services. The agency aims to reduce cancer mortality and address racial disparities in healthcare access by implementing evidence-based patient navigation, outreach, and education. The project covers support across the entire continuum of care, including identifying and addressing barriers to timely diagnostic and treatment services for priority populations. The scope of work requires the selected provider to implement patient navigation services for breast cancer screening and diagnostic imaging. For cervical cancer prevention, the provider must conduct outreach, education, and navigation to support Pap test completion and follow-up of abnormal results, focusing on communities with the lowest screening rates. Colorectal cancer screening services must include education, distribution of fecal immunochemical and fecal occult blood test kits, colonoscopy referral navigation, and follow-up support for abnormal results. The agency also requires culturally and linguistically appropriate community health education, engagement, and outreach covering cancer prevention, risk-based assessments, and triple negative breast cancer. The provider must facilitate linkage to care and financial assistance for patients. This offsite work is restricted to onshore organizations based in the United States.
