Chronic Disease Assessment, Planning and Training Service

The Colorado government authority is seeking a vendor to provide chronic disease assessment, planning, and training services. The agency aims to improve public health through evidence-based strategies that prevent, postpone, and treat chronic disease. The selected contractor must complete an implementation plan, revise this plan at least annually, and execute the approved plan. The scope of work requires the contractor to participate in the Rural Chronic Disease Consortium and submit quarterly progress reports. The agency is issuing a contract period of one year for these services. Performance of all contracted work must take place offsite. Eligible bidders must be located onshore within the United States only. The agency is currently looking for proposals to determine the budget for this project. Bidders must submit all questions regarding this opportunity no later than June 24, 2026.
