Chronic Disease Assessment, Planning and Training Service

The Colorado government authority is seeking a vendor to provide chronic disease assessment, planning, and training services. The agency aims to improve public health through evidence-based strategies that prevent, postpone, and treat chronic disease. To achieve this, the agency requires the selected provider to complete an initial implementation plan, revise this plan at least annually, and execute the approved plan. The scope of work also requires the provider to participate in the Rural Chronic Disease Consortium and complete quarterly progress reports. The agency is limiting eligibility to onshore vendors located within the United States. All contracted work must be performed offsite. The agency is establishing a contract period of one year. Bidders must submit any questions regarding this opportunity no later than June 24, 2026. The agency has not disclosed a specific budget and is currently looking for proposals to determine the cost.
