Collaborative Care Services for Perinatal Mental Health

The Albany, New York government authority is seeking collaborative care services for perinatal mental health to enhance behavioral health capacity in primary care. The agency aims to solve the problem of limited behavioral health integration by adding a Psychiatric Consultant and a Behavioral Health Care Manager to the primary care team. This collaborative model is intended to drive better clinical outcomes, increase patient and provider satisfaction, and improve functioning for patients with behavioral health needs. The scope of work requires the Behavioral Health Care Manager to have specialized behavioral health training to provide brief, evidence-based behavioral health treatment and support medication management. The provider must deliver patient-centered, treatment-to-target care that incorporates regular use of symptom monitoring tools. Additionally, the provider must regularly communicate patient status to the physical health provider and review patient progress with the psychiatric consultant to inform ongoing treatment changes. The agency is issuing a one-year contract for this project, and all performance of the work must occur offsite. Eligibility is restricted to onshore organizations based in the United States.
