The City of Denton, Texas is seeking a vendor to provide life insurance, short-term disability, and long-term disability services for its employees and eligible dependents. The agency needs to maintain its current benefit structure, which includes basic employer-paid life and accidental death and dismemberment coverages, long-term disability coverages, supplemental life, and a voluntary short-term disability program. Eligible dependents include legal spouses, declared common-law spouses, unmarried children up to twenty-six years of age, and incapacitated children of any age who depend on the employee. The scope of work requires basic life and accidental death and dismemberment benefits for two specific classes of employees. The voluntary short-term disability program must provide sixty percent of earnings up to one thousand two hundred dollars per week, featuring a fourteen-day elimination period and an eleven-week maximum payout. Bidders must perform all work offsite and must be located onshore within the United States. The agency is establishing a contract period of three years to cover these insurance and disability services.
