Forms Printing Services

The agency is seeking forms printing services for a secure power of attorney form. The required quantity is 1,000,000 forms. The forms must be four-part, carbonless, top stub, snap-out style. The overall size of the form is 8 1/2 inches wide and 11 5/8 inches high, with a torn-out size of 8 1/2 inches wide and 11 inches high. The contract period will be for one year. Eligibility is restricted to onshore USA organizations only. Performance of the work will be onsite. The agency is looking for proposals.
