Independent Incident Investigation Services

The agency is seeking independent incident investigation services for non-routine incidents that safety and risk services determine warrant third-party involvement. The project covers immediate response and mobilization upon notification, travel to and attendance at incident locations including remote sites, and incident scene examination and documentation. The scope includes identification, collection, and review of physical, documentary, and photographic evidence, as well as conducting interviews with witnesses, supervisors, and other relevant stakeholders. The agency requires the analysis of relevant policies, procedures, work practices, and environmental conditions. The investigation must apply recognized methodologies and regulatory frameworks, including a described recognized root cause analysis methodology in each report, to identify direct, contributing, and root causes. The agency expects clear, defensible, and evidence-based findings. Bidders must be an organization located in Canada. The performance of the work will be onsite. Responses can be submitted digitally via email or online. The contract period will be for one year. All questions must be submitted no later than August 17, 2026. The agency is looking for proposals.
