Caregiver Supports Program Services

The government authority located in Chicago, Illinois is seeking a vendor to provide comprehensive caregiver supports program services. The agency aims to increase awareness about the essential value of caregiving and caregiver support services while conducting ongoing outreach to ensure program participation and retention. To solve these needs, the agency requires a partner to deliver assessments, case management, outreach, counseling, support groups, memory cafes, memory and caregiver kits, training, education, and gap-filling services. The scope of work covers tailored caregiver and referral assessments, individual and family counseling, and emergency preparedness and contingency planning. The selected provider must facilitate support groups to offer a safe environment for peer exchange, host specialized support groups known as memory cafes, and distribute memory and caregiver kits. Outreach efforts must include community presentations, brochures, flyers, and social media to actively engage the caregiving community. Eligible bidders must be onshore organizations based in the United States. The agency allows the performance of the work to take place offsite. Bidders must submit their proposals digitally via email or online, and the agency is currently looking for proposal submissions to determine the budget. The agency will award a contract period lasting for two years. Interested bidders must attend a pre-bid meeting scheduled for June 12, 2026.
