RFI for Web-Based Health Risk Screening Tool

The agency is seeking information on a web-based health risk screening tool to identify and assess health and welfare risks, as well as support needs across all major life domains. This tool must evaluate health and well-being, safety and behavior, activities of daily living, decision-making capacity, and mental and emotional health. Non-clinical staff, primarily case managers, will use the tool to guide appropriate service planning and resource management. Only organizations based in the United States are eligible to participate in this market research effort. The selected vendor will perform all work offsite. Interested parties must submit their responses digitally via email or through the designated online portal. The agency is currently looking for proposals and has not disclosed a specific budget.
