RFI for Strengthen and Coordinate Services

The Washtenaw County government authority in Ann Arbor, Michigan is seeking information to strengthen and coordinate services across multiple health and community support sectors. The agency is attempting to address gaps in health and wellbeing, community living long-term services, care partner support, care management, and legal services for its target populations. The requested scope covers education programs, healthcare access, disease awareness, physical and mental health services, and insurance counseling. The project also encompasses in-home care for individuals on waitlists, friendly caller programs, adult day programs, and respite services. Additionally, the agency requires comprehensive care management models for older adults and care partners, alongside legal services covering advance directives, guardianship, estate planning, and elder abuse prevention. Performance of all requested work will take place offsite. Eligible respondents must be onshore organizations based in the United States. The agency is currently looking for proposals to determine budget parameters and has not disclosed a pre-set funding limit.
