Disability Services Navigator Consultant Grant Administered

The County of Passaic, located in Paterson, New Jersey, is seeking a vendor to administer a disability services navigator consultant grant. The agency needs to provide case consultation and navigation services for young and emerging adults, including teenagers, who have intellectual or developmental disabilities. This initiative specifically targets individuals with severe learning disabilities, Down Syndrome, Autism-Spectrum Disorder, and traumatic brain injury, as well as their families and caregivers. The scope of work requires the selected provider to deliver comprehensive case navigation services. The agency is requesting that all performance of the work be conducted offsite.
