Diabetes Self-Management Education and Support (DSMES) Programming Service

The Baltimore, Maryland government authority is seeking a vendor to provide diabetes self-management education and support programming services. This project supports the broader goal of developing a sustainable network of diabetes self-management support and education sites across the state with the capacity to offer services. The agency is trying to solve the problem of limited capacity and coordination in diabetes education by establishing structured referral processes and outreach. The scope of work requires the selected vendor to develop a provider and staff engagement plan outlining strategies and materials for engaging and training providers and staff on integration and referral processes. The vendor must deliver provider education, in-service trainings, and promotional materials for providers about referrals. Additionally, the agency requires the development of an outreach plan responsive to the priority population, active awareness and outreach with that population, and provider training using a developed operational manual. Eligibility is restricted to onshore organizations based in the United States. The agency allows the performance of the work to be conducted either onsite or offsite. The agency has not disclosed a specific budget and is looking for proposals to determine the cost. The agency has scheduled a pre-proposal meeting for July 7, 2026.
