School Nurse Led Asthma Case Management Services

The Baltimore City Health Department is seeking a vendor to provide school nurse-led asthma case management services within its jurisdiction. The agency requires a systematic method to identify and track comprehensive information about all students with asthma, including current asthma action plans, required medications, self-carry authorizations, triggers, and health room visits. To refine these processes, the selected vendor must pilot test the case management system in at least one elementary school and one secondary school. The scope of work also encompasses additional capacity-building activities as determined by the local school health services program. The agency requires all meeting facilities and conference venues to be fully accessible, with accommodations provided in a timely manner.
