The High Plains Library District in Greeley, Colorado is seeking a vendor to provide comprehensive homeless programming services. The agency is trying to solve housing instability by securing permanent supportive housing for sixty-four households. Additionally, the agency requires housing and resource navigation services, which include family reunification diversion, problem-solving, and short-term rental assistance to facilitate rapid exits from homelessness. The scope of work also covers medium-term rapid rehousing services specifically for four households. The selected vendor will perform all of the required work offsite.
