Web-Based Customizable Online Database and Workflow System

Salt Lake County, on behalf of its Health Department, is seeking proposals for a web-based customizable online database, customer relationship management, and workflow management system. The buyer requires a centralized platform that automates routine processes, tracks workflows, integrates with APIs, supports off-site mobile access, and includes public-facing data collection without requiring public login credentials. The selected contractor must provide all software, licensing, configuration, data migration, training, and technical support. Eligibility is limited to onshore USA organizations, and performance of the work will be offsite. Proposers must submit responses digitally via U3P formatted into three separate PDFs for the Program Proposal, Pricing Proposal, and Required Forms. A pre-proposal WebEx meeting is scheduled for October 21, 2026, and all questions must be submitted by October 28, 2026. The contract term is for one year. Proposals are evaluated based on factors including overall readiness, weighted at 5%, and proposed pricing, weighted at 30% using a formula where the lowest proposed price receives maximum points. The buyer is seeking pricing proposals and will not pay hourly fees for travel time.
