Clinical Knowledge Translation Services

The Vermont Blueprint for Health is seeking a vendor to provide clinical knowledge translation services to support its workforce development goals. The agency requires the development of evidence-based screening, diagnostic, and treatment protocols to improve clinical practices. Additionally, the selected vendor must design risk stratification approaches, including panel management, for various patient populations. The scope of work covers the creation of care management pathways for patients experiencing single and multiple chronic conditions. The agency also requires integration strategies for mental health and substance use disorders within the clinical framework. All performance of the contracted work will take place offsite. Only organizations based onshore within the United States are eligible to bid on this opportunity. The agency has not disclosed a set budget and is looking for cost proposals from interested bidders.
