Homeless Outreach Services

The agency is seeking a vendor to provide homeless outreach services to improve the efficiency, speed, and accuracy of engaging, diagnosing, and housing individuals experiencing homelessness. The project aims to accelerate the transition to stable housing by contacting individuals, connecting them with resource providers, and conducting follow-up to ensure successful service facilitation. The scope of work requires combining two days per week of outreach to new citizens in the tidelands area with three days per week of intense case management with known individuals in the area. This case management must focus at least in part on individuals with the longest tenures of homelessness and the most acute needs. Additionally, the selected vendor must organize a minimum of one case conference per month regarding progress and barriers, transport individuals to resource centers or referral locations, and utilize the Homeless Management Information System to enter data and provide quarterly summary reports to the agency. Eligibility is restricted to onshore, USA-only organizations, and the performance of the work will be offsite. The agency is looking for budget proposals to cover a contract period of five years. Key dates include a pre-bid information exchange meeting on June 4, 2026, and a deadline for submitting all questions no later than June 11, 2026.
