Clinical Knowledge Translation Services

The Vermont Blueprint for Health is seeking a vendor to provide clinical knowledge translation services to support its workforce development and support goals. The agency requires the development and implementation of evidence-based screening, diagnostic, and treatment protocols. Additionally, the selected contractor must provide risk stratification approaches, including panel management, for various patient populations. The scope of work covers the creation of care management pathways for patients presenting with single and multiple chronic conditions. The agency also requires integration strategies specifically designed for mental health and substance use disorders. All contracted work must be performed offsite by an organization based in the United States. The agency is looking for proposals to determine the budget for this initiative.
