Community Health Worker Program Evaluation Consultant Services

The Riverside County Department of Public Health in California is seeking community health worker program evaluation consultant services. The agency relies on community health workers to advance access to care, provide health education, support care coordination, and connect residents to public health and social service resources. To optimize these efforts, the agency needs to evaluate and improve its community health worker program infrastructure, enhance service delivery, and support long-term program sustainability.
