The government authority in Truckee, California is seeking a vendor to provide permanent supportive housing supportive services. The agency needs a low-barrier, voluntary, and flexible service model designed to support long-term housing retention for tenants. To achieve this, the selected provider must work closely with landlords and property managers to address issues that could jeopardize housing stability. The scope of work requires participating in case conferencing and multidisciplinary team coordination while coordinating care across health care, behavioral health, and social services. The provider must assist tenants in connecting to primary care, preventative services, and ongoing medical care. Additionally, the agency requires the development of individualized, tenant-driven service plans for each household and the utilization of the Homeless Management Information System to select potential tenants from the coordinated entry system. Bidders must outline a specific plan that details service providers, service locations, and hours and days of availability. The proposal must identify the staffing structure, including case managers, peer support staff, and supervisory roles, and demonstrate how the provider will use data to inform continuous quality improvement. The agency is evaluating proposals based on core supportive services, service coordination and partnerships, staffing and service availability, data collection and outcomes, and the leveraging and sustainability of funding. Eligibility is restricted to onshore organizations based in the United States. The performance of the work may occur onsite or offsite.
