Peer Outreach Services

The agency is seeking a vendor to provide peer outreach services to engage highly vulnerable populations, specifically individuals experiencing homelessness and those with a history of justice system involvement. The selected vendor will provide two full-time peer outreach specialists to conduct field-based outreach and support participant engagement and retention in services. The project aims to reduce immediate barriers to engagement and stabilization by offering flexible, client-centered support resources. The scope of work covers assisting participants with housing navigation, behavioral health and substance use treatment, and primary and specialty medical care. The peer outreach specialists will also assist with benefits enrollment, including Medi-Cal, SSI/SSDI, and CalFresh, as well as legal, reentry, and re-stabilization services. The vendor must model and apply trauma-informed, harm reduction, and culturally responsive practices while collaborating with staff to support care planning, follow-up, and warm handoffs. Additionally, the vendor must provide appropriate levels of programmatic, clinical, and administrative supervision to the peer outreach specialists. Only onshore organizations based in the United States are eligible to bid on this opportunity. The performance of the requested work will take place offsite. The agency has not disclosed a set budget and is looking for proposals to determine the cost. The agency has scheduled a pre-bid meeting for June 15, 2026.
