Caregiver Supports Program Services

The government authority located in Chicago, Illinois is seeking a vendor to provide comprehensive caregiver supports program services. The agency aims to increase awareness about the essential value of caregiving and caregiver support services while conducting ongoing outreach activities to ensure program participation and retention. These outreach efforts may include community presentations, brochures, flyers, and social media. The requested scope of work covers tailored caregiver assessments and referral assessments for case management, individual and family counseling, and support groups that provide a safe environment for peer exchange. The selected vendor will also deliver memory cafes as specialized support groups, memory and caregiver kits, training and education, emergency preparedness and contingency planning, and gap-filling services. The agency requires the performance of all work to be conducted offsite. Eligibility is limited to onshore organizations based in the United States.
