Social, Referral and Case Management Program Services

The City of Chelsea, Massachusetts is seeking a community-based organization to provide social, referral, and case management program services. The agency is trying to reduce health and safety risks by gaining early awareness of identified problems and expanding staff capacity to respond with both codes enforcement and support services interventions. The project covers addressing substance use disorders, emotional, mental, and physical distress or abuse, housing insecurity, rental assistance, food insecurity, and the need for tax preparation. The selected partner must perform case tracking and follow-up while maintaining confidential access to individual case situations. The agency requires the partner to work collaboratively with the city to balance residents' rights to privacy. Performance of the work will take place offsite. Eligibility is restricted to onshore, United States organizations only. Bidders must submit any questions regarding this opportunity no later than June 17, 2026.
