Population Health Improvement Fund Services

The Maryland government authority is seeking a vendor to provide population health improvement fund services. The agency needs to address the health outcomes of adults with high-risk pregnancies, which include pregnancies with increased risk for complications due to pre-existing conditions, new pregnancy complications, maternal factors, and fetal conditions. Applicants may also propose serving other populations with nutrition-sensitive conditions such as diabetes, prediabetes, obesity, and other cardiometabolic diseases. Eligible patients must meet the clinical criteria and screen as nutritionally at-risk, which includes being food insecure, nutritionally insecure, or having low-diet quality. The scope of work requires the vendor to establish a strong plan for building community trust in the program. The agency requires the delivery of culturally relevant produce boxes that allow for patient choice and contain a meaningful amount of produce to accomplish health improvements for the whole family and household. If an applicant proposes a project where boxes are available for pick-up instead of delivery, they must justify why this method is preferable to the community being served. Additionally, the vendor must provide support to patients in using the produce, such as recipe cards, and provide or connect patients to evidence-based wraparound services. Only onshore organizations based in the United States are eligible to apply for this opportunity. The performance of all contracted work will take place offsite. The agency has not disclosed a specific budget and is looking for proposals to define the cost. Bidders must submit all questions regarding this opportunity no later than August 3, 2026.
