Reach Crisis Care Coordination Services

The Waukesha County Department of Health and Human Services in Wisconsin is seeking a vendor to provide Reach Crisis Care Coordination services. The agency is attempting to prevent out-of-home placement for youth, improve youth coping skills to reduce the risk of harm to self or others, and assist families in obtaining supportive community services. The target population includes voluntary, Medicaid-eligible youth aged five up to twenty-three who have received a mental health diagnosis within the past twelve months. The selected vendor must provide a Care Coordinator to assist families with developing crisis plans and connecting to supportive services within the community or through the youth's insurance. The scope of work covers crisis stabilization and time-limited specialized services as part of the enrollment process. All performance of the work will occur offsite, and only onshore organizations based in the United States are eligible to bid. The agency is looking for proposals to determine the budget for these services. Bidders must submit all questions regarding this opportunity no later than July 2, 2026. The agency will evaluate proposals based on the submitted plans to fulfill the REACH program requirements.
