Rural Health Transformation Program Service

The Vermont Department of Vermont Health Access, located in Waterbury, Vermont, is seeking a vendor to provide rural health transformation program services. The agency is trying to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce, and ensure that Vermonters receive affordable, timely, and appropriate care. This grant provides funds to help health care providers implement regional care strategies that coordinate services across a geographic area to improve access, quality, efficiency, and sustainability. The scope of work requires start-up or catalytic investments that establish sustainable models of care while simultaneously addressing access, affordability, and improved health outcomes. The project covers care innovation and service redesign start-up or expansion costs, technology and interoperability, shared services and operational efficiency, and hospital designation of service transformation. Performance of the work will occur offsite over a contract period of one year. Eligibility is limited to onshore organizations based in the United States. The agency is looking for budget proposals and has not disclosed a set funding limit. Bidders must submit all questions no later than June 26, 2026.
