Healthcare Innovating Service

The Wisconsin government authority is seeking a vendor to provide a healthcare innovating service that addresses fragmented systems of care. The agency needs to target the care continuum where community members are most likely to experience barriers to health-related services or fall out of care. This funding aims to ensure that healthcare services and resources are delivered in the most efficient, accessible, and high-quality manner possible. The project scope requires establishing strong, sustainable, and community-centered healthcare delivery systems. The agency is looking to increase primary, specialty, and behavioral healthcare access while improving prevention, behavioral, and chronic health outcomes in rural communities. Additionally, the requested services must reduce avoidable hospital admissions and emergency department visits among rural residents. Eligibility is restricted to onshore organizations based in the United States. Performance of the work will take place offsite. The agency is currently looking for proposals to establish the budget for this initiative. All questions regarding this opportunity must be submitted no later than June 30, 2026.
