Cloud-Based Closed Loop Referral System

The agency is seeking a vendor to implement, host, and operate a cloud-based Closed Loop Referral System. This initiative aims to solve the challenge of disconnected community workflows by establishing a system that integrates with Electronic Health Records, Health Information Exchanges, 211 systems, Community-Based Organization platforms, and partner systems. The system must support Single Sign-On access and provide a comprehensive, interactive, real-time resource directory where individual service suppliers can update their own organization's service-related information. The scope of work covers system configuration aligned to community workflows and an implementation plan that supports both single-vendor and multi-vendor execution. The selected vendor must handle Master Patient Index identity integration, data mapping, data validation, migration testing, and migration reconciliation. Additionally, the project requires cross-platform workflow validation, a coordinated go-live strategy, and the preservation of identity and consent across systems. The agency requires the system to maintain data provenance, auditability, consistency, and the continuity of unique identifiers and Master Patient Index-linked identities. Eligibility is restricted to onshore organizations based in the United States of America. The agency requires all performance of the work to be conducted offsite.
