Homeless Evaluation, Assistance and Response Team (HEART) Outreach Services

The buyer seeks a USA-based onshore organization to provide Homeless Evaluation, Assistance and Response Team (HEART) outreach services under a one-year contract. The buyer requires welcoming, solution-focused, trauma-informed, person-centered, and culturally responsive outreach grounded in consistent relationship-building. The selected vendor must deliver street outreach and engagement, service connection and navigation, community response and field support, data collection and reporting, and required staffing. Responsibilities include assessing individual needs and barriers to connect participants to services, shelter, stabilization supports, and housing opportunities. The vendor must also develop strategies to build positive relationships with residents, businesses, neighborhood groups, and community stakeholders. Additionally, the vendor must provide accessible opportunities for electronic or written community feedback to identify recurring themes, service gaps, or education opportunities. The work will occur onsite or offsite. The buyer requests proposal pricing to establish the budget. Bidders must submit all questions by September 28, 2026.
