Community Education and Outreach Initiative Engagement Service

The Maryland agency requires community-based and faith-based organizations serving rural Maryland communities to provide community education and outreach initiative engagement services focused on advance care planning and advance directives. Selected organizations must identify at least 4-5 staff, volunteers, or leaders to participate in agency-led training, adapt outreach materials, conduct at least seven community engagement activities, and support project evaluation through data tracking and participant surveys. Eligible applicants include onshore US community-based and faith-based organizations that serve rural Maryland communities. Performers will execute the work offsite over a one-year contract period, and applicants must submit a proposal outlining their budget.
