The Indiana Department of Health Chronic Disease division seeks a vendor for a two-year agreement to provide software licenses for two online applications. Scope requirements include a branded iOS iPhone app for the National Diabetes Prevention Program and a custom app for an accredited diabetes self-management education and support program.
