Primary Accountable Care Transformation (PACT) Technical Solution for Case Management

The buyer seeks a primary accountable care transformation technical solution for case management to supplement its current technology infrastructure. The buyer requires a unified, interoperable, and configurable platform that allows state staff to oversee care management programs, enables small or remote primary care providers to participate in the PACT model, and supports justice-involved youth targeted case management. The requested tool must manage and track member care, maintain longitudinal member records, document interventions, support chronic disease and behavioral health integration, and support evidence-based chronic disease management for rural Medicaid members. The buyer limits eligibility to onshore USA organizations and allows onsite or offsite performance. The buyer is looking for proposals regarding the budget and sets a two-year contract period. Prospective bidders must submit all questions and a Letter of Intent to Respond no later than September 17, 2026.
