Brain Health Program Evaluation Services

The City of Bloomington Public Health needs an external evaluator to design and implement a 24-month prospective, equity-informed evaluation for the Bloomington Community Brain Health Services program. The chosen vendor must evaluate client-level and population-health data, assess participant outcomes and program structure from enrollment through sustained follow-up, and handle measurement, implementation oversight, analysis, and deliverables. Eligible applicants are USA-only vendors with relevant evaluation experience in community behavioral health, prospective longitudinal methods, and small programs. Vendors must register on the issuing authority website to download the unwatermarked RFP, and performance must occur onshore and onsite. Proposals are due digitally via email or the online RFP system by 11:00 A.M. on November 5, 2026, and all questions must be submitted through the online representative at least seven calendar days prior to the due date. The contract term is 24 months, with $49,900 currently available for funding alongside potential additional second-year funding. An evaluation team will score responsive proposals based on established criteria including experience, methodology, and cost.
