Mississippi Department of Health FY26 FLEX Population Health Subgrant Applications

The Mississippi State Department of Health Rural Health and Population Studies is issuing a Request for Applications for the FY26 FLEX: Population Health program. The chosen subgrantees will provide population health services in Mississippi to build capacity to achieve measurable health outcome improvements, considering social, physical, and medical care determinants of health. Eligible onshore USA entities must perform the work onsite. Applications are due by November 5, 2026, and must be submitted digitally via email or online, or physically received. Applicants must submit a detailed Work Plan and an accurate direct and indirect budget, with questions due no later than October 27, 2026. An Evaluation Committee will score responsible applications based on predetermined criteria, requiring a minimum score of 75 to receive an award. The subgrant period of performance lasts one year, with a total available funding pool of $30,000.
