The scope includes managing and tracking Money Follows the Person transitions, maintaining active client lists, and providing a case management system that satisfies data tracking and reporting requirements. Additionally, the vendor must develop, provide, and maintain an electronic system to track and assign assessments using data elements, while ensuring the messaging system allows callers to leave a callback number.
