SCOPE OF WORK: It is the mission of the John J. Pershing VA Medical Center (JJPVAMC) to provide eligible Veterans in our rural environment with high quality, compassionate healthcare; by delivering the right care, in the right place, at the right time. In keeping with this mission, JJPVAMC s Facility Management Service (FMS) is looking to partner with an A/E firm on an assessment to enhance our organization s ability to provide veteran care. This assessment will identify structural deficiencies associated with the Building 8 roof deck and, where appropriate, identify temporary safety measures to allow use of the space by JJP VAMC maintenance staff until permanent repairs can be completed. SCOPE OF WORK FOR A/E SERVICES Objective The objective of this assessment is to evaluate the current level of deterioration of the concrete roof deck and its supporting welded steel bar joists, determine whether conditions have progressed since the prior visual inspection (ref. Condition Survey Report, July 25, 2022), identify additional interim mitigation measures to allow use of building until repairs are complete (if any ,i.e. temporary protection for staff etc. ), and recommend the extent of repair required to address both immediate life-safety concerns and longer-term structural performance. Visual Inspection and Documentation Cracking Analysis: Measure and map all diagonal and horizontal cracks on the underside of the concrete roof deck, with emphasis on the southwest and northwest corners. Where feasible, compare crack patterns, widths, and extents to the prior inspection report to identify signs of progression or stability. Water Infiltration Evidence: Document areas of extensive efflorescence, water staining, corrosion byproducts, and evidence of active or historic moisture intrusion indicative of roofing membrane, flashing, or parapet cap failure. Debris Assessment: Identify locations where concrete has delaminated, loosened, or fallen, and assess the immediate risk of additional falling debris within occupied interior spaces. Concrete Deck Evaluation (Sounding & Scanning) Sounding Procedures: Perform systematic concrete sounding (hammer testing) across the concrete roof deck to identify hollow-sounding, delaminated, or unsound concrete not apparent through visual inspection alone. Reinforcement Scanning: Perform non-destructive scanning (such as Ground Penetrating Radar, GPR, an electromagnetic rebar locator, or recommend a high-resolution electromagnetic cover meters suitable for welded-wire fabric) to locate and map existing reinforcement. Condition Verification: Use sounding and scanning results, in conjunction with moisture and corrosion indicators, to evaluate whether reinforcement deterioration or loss has likely occurred in areas of advanced distress. Steel Bar Joist Integrity Assessment Corrosion Inspection: Inspect welded steel bar joists for corrosion, coating failure, deformation, and estimate section loss, particularly beneath areas of chronic leakage. Structural Viability: Evaluate whether affected joists are suitable for localized treatment (e.g., cleaning and rust-inhibitive coating), require reinforcement of chords or webs, or warrant replacement due to loss of structural capacity. Masonry and Parapet Interface Deck-to-Parapet Interface: Assess the condition of masonry parapet walls where the concrete deck is inset, with attention to moisture pathways, bearing conditions, and corrosion potential. Veneer and Joint Condition: Identify brick veneer units, mortar joints, and parapet elements exhibiting freeze-thaw damage, displacement, or deterioration requiring repair, repointing, or replacement. Reporting and Recommendations Condition and Mitigation Mapping: Prepare a consolidated plan identifying: Areas of unsound or loose concrete requiring immediate removal. Locations where short-term mitigation (e.g., debris removal, localized stabilization, or protective netting) is warranted. Areas where longer-term repair or replacement should be considered based on verified deterioration. Provide details/drawings and installation methods for each area where localized stabilization and/or protective netting is recommended. Safety and Repair Priority Summary: Provide a clear summary identifying specific areas requiring immediate repair or mitigation to reduce the risk of random falling debris, along with prioritized recommendations for subsequent corrective action based on observed and tested conditions. Follow recommendations: Provide a proposed timeline for reinspection of the items listed above if recommended. Identify the areas and frequency of follow up inspections or other actions if required until repairs are complete. Cost Estimate: Provide general cost estimates for all temporary protective measures (Including materials, installation, maintenance) and for all items requiring reinspection/follow up inspections Preliminary Layout Submission Electronic PDF copy Design Development Submission (35%): 2 Complete hardcopy sets- 1 full size, 1 half-size; 1 electronic PDF copy of drawings, specifications and cost estimate) Bid Document Submission (Final): 3 complete hard copy sets- 1 full size, 2 half size. 4 sets of bound specifications. Cost estimate. Electronic copies of drawings and specifications both PDF and Auto-CAD 2025, as well as Revit model(s).
