Senior Operations, Home-Based Services, and Care Coordination Services for Los Alamos County

The Incorporated County of Los Alamos needs a vendor to provide senior operations, home-based services, and care coordination services for older and frail adults. Scope of work includes congregate and home-delivered meals, transportation, health and wellness programs, adult day care, respite care, public benefits counseling, and social activities. The selected provider must also collect and incorporate feedback from participants and caregivers. Eligibility is restricted to USA-based onshore organizations, and work will be performed offsite. Applicants must submit a Cost Proposal Worksheet outlining all line-item costs in US dollars for up to seven years, adhering to County travel reimbursement guidelines. A pre-bid meeting is scheduled for October 29, 2026, and all questions must be submitted by November 12, 2026.
