Consulting and Facilitation Services for Community Access to Care Coalition

Grand County Public Health seeks consulting and facilitation services to synthesize 18 months of community data and develop an actionable, time-bound framework for a community access to care coalition. The scope includes discovery, facilitated goal formulation, designing and facilitating two action summits using results-based accountability workshops, facilitating memorandums of understanding or action compacts, and leading a prioritization matrix session. The maximum budget is $50,000, and the project completion deadline is November 30, 2026. Eligibility requires the organization to be onshore (USA Organization Only). Performance of the work will be offsite.
