Tompkins County Whole Health seeks one qualified USA-based nonprofit agency to implement a transitional housing and wraparound services program in Ithaca, NY. The selected organization will coordinate rapid access to transitional housing, intensive case management with a maximum caseload of ten clients per manager, and peer support for individuals experiencing homelessness and substance use disorders. Required duties include documenting client participation in a homeless management information system, delivering care, and executing transition and discharge plans toward permanent supportive housing over a three-year contract term. Proposers must submit complete operational budgets, budget narratives, reference letters, staffing plans, and signed required forms. Submissions are scored on criteria including an optional ten additional points for relevant experience in another county.
