Occupational Medical Services, Pre-Employment Examinations, and Workers' Compensation Evaluations

The City of Fountain Valley seeks proposals from qualified medical facilities and physicians to provide occupational medical screening and review services. Requested services include pre-employment examinations, comprehensive physical exams for sworn safety and field services candidates, short physicals for office personnel, workers' compensation injury evaluations and treatment, and tuberculosis testing. All services must comply with regulations from agencies such as Cal OSHA, the Division of Workers’ Compensation, and the Americans with Disabilities Act. Bidding is open to USA organizations, and the work will be performed offsite. Applicants must submit questions via PlanetBids by November 16, 2026, at 4:00 p.m. PST. Final proposals must be uploaded electronically in PDF format to PlanetBids by December 8, 2026, at 4:00 p.m. PST. Submittals must include separate response and cost files, an equal opportunity certification, current malpractice insurance policy certificates, standardized evaluation forms, and contact details for at least two references. Proposed fees should be effective July 1, 2027, and cover the entire contract term. The selected vendor must obtain a valid City of Fountain Valley Business License.
