- Therapists/Pathologists (DT, OT, PT, SLP) must have graduated from an accredited school with necessary degree
- Otolaryngologist, Audiologist, Ophthalmologist, or Optometrist must have graduated from accredited school with relevant degree
- PA or APRN must have graduated from an accredited school or program
- Psychologist (EdD, PhD, PsyD) must have graduated from accredited psychology school with appropriate post-graduate training and experience
