Rural Health Transformation Auxiliary Mobile Services for Underserved Washington Counties

The Olympic Area Agency on Aging seeks contractors to provide rural health transformation mobile services delivering preventative and auxiliary healthcare to remote and underserved areas across Clallam, Grays Harbor, Jefferson, and Pacific counties. Selected vendors must provide one or more designated services, which include connecting residents with chronic disease screening and self-management programs, naloxone and opioid prevention education, mental health screenings, dental services, telehealth referrals, foot care, and consumer wearables.
