The buyer seeks a vendor to provide care enrollment and outreach services to seamlessly connect patients to healthcare. These services include providing a primary care provider, access to specialty care, behavioral health services, prescription drug benefits, and 24/7 customer service. The selected partner will screen applicants through the State Marketplace, facilitate direct enrollment for new or renewing members, connect individuals with special circumstances to financial counselors, and conduct digital and in-person outreach in select TRIE zip codes. The partner must also refer eligible individuals to care authorized enrollers for appointment scheduling.
